New Students Requirements

Required information for new enrolments at Rawlinson Primary School:

    Child’s Birth Certificate/extract or passport or travel documents.

    Proof of address: one of these listed items only:

    • Current lease agreement or
    • Mortgage agreement or
    • Current rates notice (water or shire) or
    • Statutory Declaration, signed by a Justice of the Peace which declares parents full name and address.

    Immunisation history statement. Available online at medicare request statement or phone 1800 635 809, alternatively if you have linked Medicare to your MyGov account, you can download directly from there. If your child is starting Kindergarten, the Government requires an up to date immunisation history statement (within the last 2 months), or an exemption certificate. Your child is not enrolled until this is provided

    Proof of parent’s Citizenship/Permanent Residency or Visa number, entry date and Visa Grant Number – if either parent was born overseas
