Required information for new enrolments at Rawlinson Primary School:
1. Child’s Birth Certificate/extract or travel documents.
2. Proof of address: Proof of home ownership such as rates notice, Lease agreement, Mortgage documents or Statutory Declaration, signed by a Justice of the Peace which declares parents full name and address.
3. Immunisation history statement. (Immunisation Status must show “Up to date”) Available by logging into your Medicare online account, or Online at Medicare Australia request statement or phone 1800 635 809. Dated within 2 months of Enrolment.
4. Proof of parent’s Citizenship/Permanent Residency or Visa number, entry date and Visa Grant Number – if either parent was born overseas.
