Apply to Enrol

Required information for new enrolments at Rawlinson Primary School:

1. Child’s Birth Certificate/extract  or travel documents.

2. Proof of address: Proof of home ownership such as rates notice, Lease agreement, Mortgage documents or Statutory Declaration, signed by a Justice of the Peace which declares parents full name and address.

3. Proof of parent’s Citizenship/Permanent Residency or Visa numberentry date and Visa Grant Number – if either parent was born overseas.

4. Immunisation history statement (Immunisation Status must show “Up to date”) that is not more than 2 months old when you accept the offer of a place.  Available by logging into your Medicare online account, or Online at Medicare Australia request statement or phone 1800 635 809.
